Please provide your name, contact details, the patient’s name and age, and a brief description of the reason for the appointment. Please do not send detailed medical records or urgent clinical information by website form unless requested by the practice.

Referrals

Some patients may be referred by a general practitioner, paediatrician, physiotherapist, hospital, emergency unit or another healthcare provider. If you have a referral letter, imaging report, X-rays or other relevant documents, the practice will advise how these should be provided.

Medical Emergencies

The practice website and contact forms are not for emergencies. If your child needs urgent medical attention, please contact emergency services, your nearest emergency unit, or an appropriate healthcare provider immediately.

Consent and Minors

Because the practice treats children, a parent, legal guardian or authorised caregiver may be required to provide consent for appointments, communication, treatment, procedures and the processing of personal information. The practice may request proof of authority to consent where appropriate.

Confidentiality

Patient information is handled confidentially and in accordance with applicable law, healthcare obligations and professional standards. Information may be shared with other healthcare providers, hospitals, administrators, medical schemes or service providers where necessary and lawful for treatment, administration, billing, referral, record keeping or legal compliance.

Fees, Billing and Medical Schemes

Fees, billing arrangements and medical scheme processes should be confirmed directly with the practice before your appointment. Medical scheme cover is not guaranteed and remains subject to the rules and authorisations of your medical scheme.

Cancellations and Late Arrivals

If you need to cancel or reschedule an appointment, please contact the practice as soon as possible.
